Form 34 is the first step to claim benefits from PERKESO. Without submitting it, SOCSO cannot process any medical or compensation benefits. How to Fill in PERKESO Form 34

The form is divided into sections, and the required parts depend on the type of incident being reported (accident, occupational disease, permanent disability, or death). Based on SOCSO’s official Guide to Filling Form 34, here are what you need to do:

  1. Select the notice type. Is it accident, occupational disease, permanent disability, or death?

  2. Section A: Employee Details (Insured Person). Fill the full name, IC number, date of birth, address, occupation, and contact details.

  3. Section B: Employer Details. Fill the company name, employer code, company address, and phone number.

  4. Section C: Employee Wages. Mention the salary details for the past six months prior to the incident, required for benefit calculation.

  5. Section D/E/F/G: Incident or Illness Details. It depends on the claim type:

    • Section D: Accident details (date, time, place, cause).

    • Section E: Occupational disease details.

    • Section F: Permanent disability details.

    • Section G: Death details (including cause and date of death).

  6. Section H: SOCSO Office. Indicate the SOCSO office handling the case.

  7. Section I: Employer Certification. Signed and stamped by the employer (mandatory for accident and death cases).

  8. Section J: Employee/Dependent Declaration: Signed by the insured person, dependent, or representative. Include active bank account details for benefit payments (no joint accounts allowed).

Use black or blue ink and make sure all information is complete and accurate to avoid delays.

  • Completed Form 34

  • Copy of the insured person’s IC (front and back)

  • Medical certificate or medical report

  • Police report (for road traffic accidents)
