How to Check SOCSO / PERKESO Number

Employers can verify SOCSO numbers using three main methods based on official PERKESO guidelines:

1. Check SOCSO Online (e-Semak Portal or MyPERKESO App)

The fastest way is through the e-Semak portal:

  1. Open your browser and go to http://esemak.perkeso.gov.my or visit www.perkeso.gov.my and click on “e-Semak”.

  2. On the homepage, choose either Employer or Employee:

    • For employers: enter the company name or employer code.

    • For employees: enter the employee’s IC number (without dashes).

  3. The system will display the registration status instantly.

Employers and employees can also check through the MyPERKESO mobile app, which offers the same features.

2. Check SOCSO on Payslip

Many companies print the SOCSO number and contribution details directly on employee payslips. If the number is not listed, employers can cross-check it in their HR records or through the online portal.

3. Check SOCSO by Calling the Hotline

Employers can contact the PERKESO hotline at 1-300-22-8000 for assistance. Prepare the employee’s IC number or employer reference number for verification when calling.

How to Make a SOCSO Claim

If an employee suffers a workplace accident or illness, employers must help them file a SOCSO claim promptly. The basic steps are:

  1. Report the incident to SOCSO immediately.

  2. Prepare and submit the required forms:

    • Accident report (Form 21) or occupational disease report (Form PKS 68/69).

    • Claim form (Form 10).

    • Medical reports, attendance records, and employer’s identification letter.

    • Police report and travel sketch (for commuting accidents).

  3. Submit documents online via the Assist Portal or at the nearest PERKESO office.

  4. Follow up with SOCSO to check claim status.

Medical treatment at SOCSO panel clinics or government hospitals is free once the claim is approved. If treatment is done at non-panel clinics, reimbursement claims must be filed with additional receipts and reports.
